I, the undersigned am the parent/guardian of the registrant, a minor, and agree that the registrant and I will abide by the rules of the Merrimac Youth Baseball, Inc.  I give approval for above registrants participation in any and all league activities during the 2008 season.  I assume all risks and hazards incidental to such participation, including transportation to an from all activities and I do hereby waive, release, absolve, indemnify and agree to hold harmless the Merrimac Youth Baseball, Inc., the organizers, sponsors, participant’s, coaches, volunteers, umpires, board of directors and any persons transporting my child to or from all activities, from any claim arising out of any injury to my child, whether the result of negligence or from any other cause, except to the extent and in the amount covered by the accident or liability insurance of the Merrimac Youth Baseball, Inc.  I understand that the Merrimac Youth Baseball, Inc.  Medical Insurance is excess over any primary coverage issued to the above registrant, and that I am responsible for any deductible or other amounts not covered by insurance.

 

Unsportsmanlike behavior by Player OR Player’s Parent may result in Expulsion from

Merrimac Youth Baseball, Inc., activities.
